FIRST:  To insure that you will get credit for this elective rotation, you must have an affiliation agreement signed between your school and Boards Ready.  Please contact your Clinical Dean for this form.  You have a choice of a 4 week elective rotation in either Family Medicine or Urgent Care.

Once an affiliation agreement has been signed, please fill out the registration form below
and hit submit.
Then go to the "Make A Payment" page and submit your deposit for the rotation and the apartment deposit (if you are planning to use the Boards Ready apartments).
We can not hold your rotation spot until all deposits are made.
